Introduction

Coronavirus Disease – 2019 (COVID-19) poses a serious threat to global health and economy, and has been declared as a pandemic by the World Health Organisation (WHO). This questionnaire is aimed at assessing the knowledge attitude and practice towards COVID-19 in Nigeria. The information you provide will be anonymised and treated confidentially. However, the outcome of this study may be transmitted to the local and/or national government for policy formulation.
